Register
First time users are asked to register with the WVADR by providing physician contact information including:
- name
- mailing address
- e-mail address
- WV Medical License Number.
To register, enter WVADR database and click NEW REGISTRATION at the top right of the screen.
After verification of the Medical License Number, physicians will receive an e-mail with a user name and computer generated temporary password. Once you log in to the database and use your temporary password you will be prompted to change the password. Your password should be at least 6 characters long and contain both upper and lower case letters, numbers and at least one special character (e.g., ! @ # $ % ^ & * ? > < { } [ ] \ | ` ~ + = - _ : : “ ‘)
Helpful hints about the web-based WVADR form
- Please turn off popup blockers on your internet browser. Popups are required and need to be enabled for this website. You will find further instructions once you enter the site (POPUP WINDOWS HELP).
- When you enter the site you will see a Message system that allows two-way communication between you and WVDR. This is NOT a regular e-mail system that will let you contact people outside WVADR.
- There are tabs that allow you to register a patient, view, download and print a user manual (I Need Help) as well as view, download and print Frequently Asked Questions (FAQs).
- When entering patient data please complete all 24 questions before clicking the red “Submit” button at the bottom of the screen. Once the Submit button has been clicked a warning will appear asking you to confirm your entries. If confirmed, the data will be locked and can no longer be changed.
- Questions with a red asterisk require an answer and the form will not be complete until all asterisked questions are answered.
- Questions with a red “x” button
will allow you to clear your answer to that question by pressing the button.
- There are blue “question” buttons
that will provide additional information. This information as well as answers to other questions can also be found on the FAQ pages.
- If you need to finish filling the form later or are called away from the screen please click the red “Finish Later” button at the bottom of the screen. The screen will automatically time out and exit after 30 minutes.
- There is pull-down menu for data management that allows you to view your patient data, complete incomplete records and search for patient records. A pull-down menu for Program Management allows you to change your password.
